How to submit a referral form?
To upload a referral form, please follow these steps:
1. Log in to your portal.
2. Navigate to the "Profile" tab.
3. Locate the button labeled "Primary Care Referral" and click on it.
4. Select the "Upload" button to proceed.
5. Fill in the necessary information as prompted
